Privacy Policy

Stat-Med, Inc. is committed to protecting the privacy of those who entrust us with Protected Health Information, as required by the Department of Health an Human Services Privacy Regulations outlined in the Health Insurance Portability and Accountability Act of 1996, 45 CFR Parts 160 & 164 (HIPAA Privacy Regulations).

Scope

This policy sets forth the privacy principles that STAT-MED follows with respect to transfers of Protected Health Information ("PHI") collected or received by STAT-MED in conducting and/or managing our day to day business.

Definitions

"Customer" means any facility or facility personnel, pay-source or pay-source personnel, or anyone acting on behalf of a facility or pay-source.

"Agent" means any third party that processes PHI on behalf of and under the instruction of STAT-MED.

"Protected Health Information" ("PHI") means any information or set of information that identifies or can reasonably be used to identify an individual. PHI does not include information that is encoded or made anonymous, or publicly available information that has not been combined with non-public personal information.

Security

STAT-MED will take reasonable precautions to protect PHI in its possession from loss, misuse and unauthorized access, disclosure, alteration and destruction. STAT-MED endeavors to limit access to PHI to personnel that have a specific business purpose for maintaining and processing such PHI. STAT-MED personnel who have been granted access to PHI are aware of their responsibilities to protect the security, confidentiality and integrity of such information.

Transfers to Agents

STAT-MED will only transfer PHI to an agent where the agent has provided assurances that the agent provides at least the same level of privacy protection as is required by these principles. Should STAT-MED become aware of an agent using or sharing PHI in a way that is contrary to these principles, STAT-MED will take reasonable steps to prevent or stop such activity.

Access

STAT-MED agrees to provide customer with reasonable access to Protected Health Information via our web-site, www.stat-med.net, providing customer agrees to protect such information as required by the Department of Health an Human Services Privacy Regulations outlined in the Health Insurance Portability and Accountability Act of 1996, 45 CFR Parts 160 & 164 (HIPAA Privacy Regulations).

STAT-MED provides access by means of an agent providing web-site service with which STAT-MED has a signed business agreement outlining required HIPAA Privacy Protections.

STAT-MED requires a unique user name and password (log-on credentials) for each customer or customer site.

Customer Responsibilities

Customer will take reasonable precautions to protect PHI in its possession from loss, misuse and unauthorized access, disclosure, alteration and destruction. Customer endeavors to limit access to PHI to personnel that have a specific business purpose for maintaining and processing such PHI. Customer personnel who have been granted access to PHI are aware of their responsibilities to protect the security, confidentiality and integrity of such information.

Customer is required to protect the log on credentials, ensuring only those individuals who need access to PHI possess the log on credentials. Customer will change log-on credentials as needed to maintain access limits to personnel that have a specific business purpose for maintaining and processing such PHI.

Log-On credentials may be changed by an authorized user by changing the password. Customers will need to update user names and/or passwords with customer personnel changes. User names may be added, changed, or deleted by contacting STAT-MED staff at 623-434-4895.

Online Privacy

Please be aware that Internet transmissions, to include emails, are never totally secure. Information available on this site via the Internet or through email may not be secure; however, we will take reasonable steps to protect any PHI from unauthorized access and misuse. STAT-MED cannot guarantee or warrant this website or its security components against breaches of security, the interruption of the system or user error and misuse.

This site may contain links or references to other websites. Please be aware that we do not control other websites and that, in any case, this Privacy Policy does not apply to those websites.
